SAMPLE QUESTIONS

1. A resident asks for the bathroom door to stay open during bathing. What should guide your response?

Privacy and dignity still apply. Close the door or curtain as much as the task can be done safely, and explain what you are doing. The principle is resident rights, not speed.

2. You notice a new red area on a resident’s heel while helping with a bath. What is the nursing assistant’s next responsibility?

Finish the safe part of the care, then report the observation promptly to the nurse. Do not decide a diagnosis. The principle is observation and reporting.

3. Before you help a resident move from the bed to a chair, which check comes first?

Confirm the plan, lock the equipment, and make sure the resident’s feet are positioned for the move. The principle is safety before the transfer, not after it.

4. A question says “the resident refuses.” What should you watch for in the wording?

Refusal changes the action. Forcing care is not the answer. Explain, offer a choice when you can, and report the refusal. Words such as “first,” “best,” and “refuses” change which principle applies.
